Oasis Pelvic Health Support - Out-of-Town Program Application

Oasis PT & Pelvic Health offers a personalized, intensive program for patients traveling from outside the DMV area. Whether you’re visiting from another city, or country, we will design an integrative plan based on your individual needs.

Step 1 – Apply

Fill out our Out-of-Town Program Application. A team member will contact you within 24 hours.

Step 2 – Deep Dive Share

You will receive an email with instructions to complete an extensive pelvic floor history form, due within 48 hours, along with a deposit.
Your 90-minute online session will be comprehensive. You will leave with valuable insights that can immediately impact your healing journey.

Step 3 – Plan

Following your initial session, you and your therapist will decide next steps, including additional online sessions and/or planning your intensive, in-person treatment program at our Bethesda, MD office.

Step 4 – Begin

When you arrive, you’ll begin your personalized treatment plan with our team. Multidisciplinary and complementary therapies included pending indication, along with long-term recommendations.
